CNSiQ consists of a series of simple-to-understand questions which covers the domains of general physical disorders, mental health, substance use, and sleep related disorders, their presentations and symptoms.

Questions are designed using a scale from zero (0) to ten (10), “Zero” indicating absence of that symptom or issue and “Ten” indicating strong and severe presence of that symptom or issue
